If you are a Wireless Telecommunications Construction or Engineering firm that is interested in participating in one of our current or upcoming projects, please ensure that you meet our Vendor Qualification requirements and request a Vendor Meeting by emailing vendors@avengerengineering.com. Below is a list of all of the documents that we require for the Vendor Approval process:

  • Copies of the executed Master Services Agreement form, including all attachments and exhibits.
  • Certificate of Insurance in accordance with the provisions in Exhibit A.
  • Additional Insured endorsement for General Liability (Form CG20101185)  or  equivalent.
  • Copy of executed Subcontractor Safety & Health program commitment letter.
  • Evidence of Workers Compensation Insurance
  • W-­9
  • Experience Modification rating of 1.0 or lower
  • OSHA 300 Log Summaries
  • Rescue  Certifications
  • Safety Manual Requiring 100% Tie-­Off
  • State Licenses*1
  • Minority / Veteran Certifications*2
  • Trade  Certifications*2
  • Tower Safety & Rescue Certifications*3

*1 Avenger Engineering Vendors are required to maintain adequate licensing as mandated by any state/municipality in which they wish to work.

INSURANCE

Insurance Coverage. Without limiting Contractor’s indemnity obligations, Contractor will maintain the following

Minimum insurance coverage during the term of this Agreement and performance hereunder:

a) Commercial General Liability with minimum limits of

  • $2,000,000 Annual General Aggregate
  • $2,000,000 Products-Completed Operations
  • $2,000,000 Personal and Advertising Injury,
  • $1,000,000 Each Occurrence.

Coverage will include:

i. Any person or organization acting on behalf of Contractor;

ii. Contractual Liability;

iii. Products/Completed Operations will be extended or renewed to remain in force for a minimum of two

(2) years after substantial project completion;

iv. Bodily Injury and Property Damage resulting from professional services;

v. Property in Contractor’s care, custody, control or possession;

vi. Per Project Aggregate;

b) Commercial Automobile Liability with minimum Combined Single Limits of $1,000,000 Each

Accidental Coverage will include:

i. Owned (if applicable), hired and non-owned vehicles (or ‘Any Auto’);

ii. Any vehicle required to be licensed under the applicable vehicle code;

iii. Motor Carrier Endorsement MCS-90.

c) Workers’ Compensation and Employer’s Liability in compliance with statutory and federal laws and requirements of the hiring and working states with minimum Employer’s Liability limits of

  • $1,000,000 Each Accident;
  • $1,000,000 Disease Policy Limit;
  • $1,000,000 Disease Each Employee.

d) Commercial Umbrella/Excess Liability with minimum limits of $5,000,000 Each Occurrence and $5,000,000 Aggregate. Coverage will include terms and conditions on a following form basis in excess of the underlying coverage for Commercial General Liability, Commercial Automobile Liability and Employer’s Liability.

e) Professional Liability (Errors & Omissions) insurance, if applicable, with limits of at least $1,000,000 each claim or wrongful act, and at least $2,000,000 in the aggregate for Architects and/or Engineers.

“Subcontractor agrees to maintain such insurance for a period of at least two (2) years following the completion of this contract.”

f) “All Risk” or “Special Form” Builder’s Risk and/or Installation Floater with a minimum Each Loss limit being the maximum exposure for the largest single project. Coverage will include property hoisted, stored, transported, or otherwise in Contractor’s care, custody and control.

g) “All Risk” or “Special Form” Property with minimum Each Occurrence limits being one hundred percent (100%) of the replacement value of the property in Contractor’s care, custody, control, or possession. Coverage will include:

i. Perils of flood, earthquake and windstorm;

ii. Business Interruption and Contingent Business Interruption;

iii. Waiver of any co-insurance requirements;

iv. Property hoisted, stored, transported, or otherwise in Contractor’s care, custody and control.
